Influence of stage nursing intervention on amputation patients

Phased nursing refers to the psychological, pain and rehabilitation characteristics and needs of patients at different stages, targeted and focused to provide phased, systematic and continuous nursing intervention for patients to adapt to the development of patients' diseases and meet the needs of patients.

Amputation is a destructive operation applicable to severe limb infection, trauma, lung tumor, vascular disease, nerve injury and limb deformity, among which accidental trauma is the most common. Due to the suddenness and unexpectedness of the trauma, the limb function and body image of the patient have changed after the injury, and the patient cannot accept and adapt to this change physically and psychologically, which leads to a series of problems such as fear, tension and phantom limb pain, seriously affecting the patient's recovery. Periodical and continuous nursing intervention is of great significance in alleviating patients' negative psychological stress, helping patients recover their ability to live daily and adapting to changes in body image and function.

 

 

Amputation caused by trauma can be sudden and accidental. Patients in unprepared circumstances, sudden loss of limbs, pain, body changes and functional limitations to the patient's psychological and physical impact; In addition, the initial trauma of tension, anxiety, preoperative fear, recovery of depression, pessimism torture patients' mind, some patients even appear world-weary, suicidal tendency. Conventional amputation nursing is limited to drug analgesia, simple comfort and health education, far from meeting the physiological and psychological needs of patients.

 

 

The important role of stage nursing: amputation of upper limbs and forearms is mostly seen in young and middle-aged male manual laborers. If limb disability seriously affects the livelihood of patients, amputation is undoubtedly a heavy and fatal blow to them. Psychological intervention is of great significance in stabilizing patients' emotions, guiding them to get out of their "psychological despair", helping them to establish the belief of overcoming the disease, and improving patients' treatment compliance. According to the psychological characteristics of patients with forearm amputation, the psychological process of patients is divided into three stages, namely anxiety, tension stage, fear stage and depression, pessimism stage, according to the psychological characteristics and psychological problems of different stages, to provide patients with phased, continuous nursing intervention. After intervention, SAS, SDS and fear scores were significantly decreased (P < 0.01). It can be seen that stage psychological nursing is more in line with the psychological characteristics and psychological needs of amputee patients, which is particularly important to relieve patients' tension, anxiety, fear and stabilize their emotions. Pain in patients with forearm amputation can be divided into acute pain, chronic pain and phantom pain. The major forms of pain in amputees vary at different times. Most patients had acute pain before surgery, but most patients had chronic pain after surgery, and some patients also had phantom limb pain after surgery.

 

 

According to the characteristics of different forms of pain in different periods, diversified analgesic methods can be used, such as body position nursing, drug analgesia, electromagnetic wave analgesia, physical analgesia, progressive nerve, muscle relaxation and distraction, etc., to achieve better analgesic effect. As shown in Table 2, PPI, VAS and PRI scores of patients were significantly reduced after intervention (P < 0.01). The results showed that staged pain intervention was of great significance in relieving pain degree and intensity.

Rehabilitation training is the first and most important step for amputees to return to their families and communities. Phased rehabilitation training fits the rehabilitation process of patients and pays more attention to practicability, which helps to enhance patients' belief in rehabilitation and improve their quality of life. It is of great significance to relieve patients' anxiety, depression and fear, effectively control pain and improve patients' quality of life.
